To determine eligibility or coverage (e.g., verifying benefits with your insurer)
C. Health Care Operations
For quality improvement, staff training, compliance reviews, or accreditation
To contact you about appointments, treatment reminders, or clinic updates (via phone, email, or mail—unless you ask us not to)
D. Other Permitted or Required Uses
We may also disclose PHI when required by law, such as:
Reporting certain injuries (e.g., suspected abuse or gunshot wounds)
Responding to public health authorities
Complying with court orders or subpoenas

Inspect and obtain a copy of your health records (we may charge a reasonable fee for copying)
Request an amendment to your records if you believe information is incorrect or incomplete
Receive an accounting of disclosures (a list of certain non-routine disclosures of your PHI made in the past 6 years)
Request restrictions on how we use or disclose your PHI (we are not always required to agree, but we will consider your request)
Request confidential communications (e.g., ask us to call you only at a specific number or send mail to a different address)
Receive a paper copy of this Notice, even if you agreed to receive it electronically
